Xiomara Pocasangre: Improving Health through Quality

Quality Improvement Specialist, Xiomara Pocasangre knew she wanted a meaningful career and what started as a referral for a great opportunity turned into a memorable career at HealthPoint. As a Quality Improvement Specialist (QIS), Xiomara works closely with every clinic to improve the health outcomes of the patients. The Quality Team prioritizes Quality Improvement (QI) workaround preventive screenings such as breast, colon, and cervical cancer and chronic condition management such as Diabetes and Hypertension. The goal of HealthPoint’s quality team is to help streamline processes to improve the health outcomes of the communities the health clinics serve. Xiomara continues, “Our team has a quality guidance that outlines the expectation for quality improvement at the clinics such as a continuous test of change (PDSAs), analyzing results, adopting new processes, and documented workflows to ensure full adoption of changes at the site level.”  

In 2017, Xiomara Pocasangre and other HealthPoint staff noticed a lot of refugees were settling into the King County area, many of whom were pregnant women and saw patterns of social isolation, and a lack of community building and support. HealthPoint providers searched for ways to help and learned of Centering, a model of care provided in a group setting to pregnant women. Xiomara’s role expanded to include Centering and ways to improve pregnancy outcomes within the community. Xiomara’s biggest inspiration is her mother, “She has supported me and motivated me to continue to achieve my goals in life and also is a great role model as a mother, I aspire to be a great mother like she is.”
