We are happy you’ve chosen HealthPoint for your health care. Every day, we follow thorough cleaning and safety procedures to keep our patients safe and healthy.

How to find care

We are accepting new patients. You can find a clinic from our list of clinic locations, or search for the type of care provider  you want to see. Then call the clinic phone number to make your appointment.

Current patients can also request an appointment on MyChart.

Telephone and in-person appointments


In order to best protect our patients and staff, HealthPoint is limiting in-person clinic appointments to the following:

  • Obstetric care

  • Children 6 years and under

  • Suboxone treatment

  • Dental care - Emergency and routine care is available. Appointment availability varies by clinic.

  • Acupuncture and Osteopathic Manipulative Care - Appointment availability varies by clinic.

  • Some procedures such as IUD placement

Most other appointments will be telephone or virtual visits. Please make your appointment using MyChart or call before coming to a HealthPoint clinic.

What to bring

To save time, complete these forms and bring them with you to your first appointment.

You should bring to your appointment:

  • Your driver's license or other identification 
  • Medications you are taking
  • Your insurance card (if you have one)
  • Any medical records you have
  • If you are applying for sliding fee, please bring proof of the last 30 days of your income
     

Need to cancel an appointment?

It’s important to let your doctor know if you can’t come to your appointment. Call your clinic if you need to cancel your appointment.

Arrive early for your appointment

At your first appointment, please arrive 30 minutes before your appointment time to complete your paperwork.

At future appointments, please arrive at least 15 minutes before your scheduled appointment time. It is not always possible for your doctor to see you if you arrive late to your appointment.

What should I expect when I arrive at HealthPoint?

If you have an in-clinic appointment, please try to come alone. If possible, do not bring family or friends who are over age 60, pregnant, or have health conditions like heart disease, asthma, or diabetes. This is to protect the health of your family and friends, and to limit the number of people in our clinic lobbies.

Hand sanitizer is available throughout our clinics. Please use sanitizer when you arrive and before you leave. Wash your hands after using the restroom or if they are dirty. Our providers and staff wash their hands and use sanitizer before and after seeing every patient.  

Don’t have an appointment and need urgent care?

Need non-emergency medical care in the evening or on weekends? You can walk in to our Kent Urgent Care location without an appointment for treatment of minor illnesses and injuries. If it is a life-threatening emergency, call 911.
