Patient and Family Advisory Committee
Improve Care. Build Community.
The Patient and Family Advisory Committee is a volunteer group of HealthPoint patients, family members, patient care takers, and community partners. The goals of the Committee are to:
- Improve the patient and family experience at HealthPoint
- Support our communities
- Build strong relationships with each other
- Find solutions together to improve health and wellbeing
PURPOSE:
The Patient & Family Advisory Committee (PFAC) will have an active role in improving the patient and family care experience at HealthPoint and strenghtenting our communitues.
PFAC RESPONSIBILITIES:
- Attend meetings on a regular basis
- Serve in an advisory capacity to HealthPoint, not as a decision-making body
- Discuss patient, patient family/caretaker, and community concerns/needs
- Act and speak with the goal of improving patient and family members’ care
- Prepare for meetings and discussions by reading materials sent before meetings
- Provide feedback, both positive and negative, in a constructive way
- Abide by the Code of Conduct
- Be willing to serve on PFAC for a minimum of one year
MEMBER SELECTION:
- A patient, family member, or patient caretaker may join independently or be referred to join by a HealthPoint staff member. For more information, please contact pfac@healthpointchc.org
